Custom Forms: Consent, Intake & Discharge Documents
The Form page in Settings lets the clinic build custom paperwork — surgical consent forms, intake questionnaires, discharge instructions, vaccination records, anesthesia checklists, and anything else that used to live on a clipboard. Forms can be filled out on a tablet, signed digitally, and saved straight into the pet’s medical record.
How to access
- From the main menu, click Settings.
- Click the Form tab.
- Filter by category (consent, intake, discharge, anesthesia, etc.) or by active/inactive.

Building a new form
- Click + Add Form.
- Fill in:
- Form name.
- Category.
- Trigger — when the form should pop up (e.g., before a surgery is started, at registration, at discharge).
- Drag fields from the field palette into the form:
- Text — short or long text input.
- Number.
- Single choice / Multiple choice.
- Date / Time.
- Image upload.
- Signature — touchscreen-friendly signature pad.
- Section header — visual divider.
- Static text — for clauses, legal language, instructions.
- Mark fields as required or optional.
- Add merge variables to static text so the form pre-fills with owner name, pet details, etc.
- Click Preview, test on a tablet, then Confirm.

Using a form in practice
- From the medical record, click Forms.
- Pick the form to fill out.
- Hand the tablet to the owner if a signature is needed.
- The completed form saves automatically into the pet’s record as a PDF.

Multi-language forms
Each form can have language variants. The system serves the variant that matches the owner’s preferred language, falling back to the clinic default. Useful for clinics that serve a multilingual community.

Editing or retiring a form
- Click the form to edit. Changes apply only to new instances — previously signed forms stay frozen.
- Use Set Inactive to retire. Historical signed copies remain in the medical records.

Tip
Run new consent forms past your local legal advisor before deploying. The form builder is flexible, but the language and required disclosures in a surgical consent vary by jurisdiction. A 30-minute review with counsel saves headaches later.
